You went to your doctor.
You asked for answers.
They ran the labs.
They reviewed the results.
They looked you in the eye and said: everything looks normal.
And, you went home still exhausted, still bloated, still foggy, and still struggling to perform at the level you know you’re capable of.
If this has happened to you, maybe even more than once, you are not imagining it.
You are not anxious.
You are not someone who just needs to manage stress better.
You are someone whose body is communicating clearly, and whose conventional labs are not designed to hear what it’s saying.
Here’s why.
What Conventional Labs Actually Measure
Standard bloodwork is designed to catch disease, not to measure function.
The reference ranges on your labs are built from population averages, which means “normal” is defined as whatever most people look like.
Most people are not well.
They are simply not sick enough to flag.
When your TSH comes back at 3.2 and your doctor says your thyroid is fine, what they mean is: you do not have clinical hypothyroidism. What they don’t mean is: your thyroid is functioning optimally for your body.
When your ferritin comes back at 14 and they say your iron is normal, what they mean is: you are not clinically anemic. What they don’t mean is: your cells have the iron they need to produce energy efficiently. Functional practitioners consider ferritin below 50 a problem. Below 30 is where hair loss, fatigue, and cognitive fog become predictable.
When your liver enzymes come back in range, what that tells you is: your liver is not in acute distress. It tells you nothing about whether Phase I and Phase II detoxification are running efficiently, whether bile flow is adequate, or whether your body is actually clearing the hormones, toxins, and metabolic waste it accumulates every single day.
I want to be clear: I believe deeply in testing.
I’m an early angel investor in Function Health (MSWIDLER10 to skip the waitlist) because I think comprehensive lab data is one of the most powerful tools we have for understanding what’s actually happening inside your body. I run my own labs twice a year, and have done so for many years at this point.
I also use the Vibrant Wellness Total Tox Burden Test and Organic Acids Test as well as the Tiny Health Gut Health Test (MEGHAN for $20 off) on myself regularly.
Lastly, I have also run select add-on tests on myself, including the Alzheimer's gene detection test, the GRAIL Galleri multi-cancer early detection test, and the Ezra full-body MRI, all through Function Health, as well as the breast cancer ultrasound through BeSound (MEGHANS50 for $50 off your first scan).
But, here’s the distinction that matters: there is a significant difference between standard bloodwork and functional comprehensive lab testing.
The issue is not testing itself.
The issue is testing the wrong things, and then interpreting results against population averages rather than optimal ranges.
The Gap Between “Normal” and Optimal
There is a wide valley between “not diseased” and genuinely well.
Conventional medicine lives at one end of that valley.
It is looking for pathology.
When it doesn’t find pathology, it sends you home.
What it rarely measures:
Intracellular nutrient status, not serum levels, which reflect what’s circulating, but what’s actually inside the cells doing the work.
Organic acids, which reveal how efficiently your mitochondria are producing energy and whether your detox pathways are running.
Comprehensive stool analysis, which shows what’s living in your gut, what’s missing, and how well you’re actually absorbing what you eat.
Toxic burden, which reveals the cumulative load of heavy metals, mold mycotoxins, environmental chemicals, and pesticides your body is carrying and trying to process.
Hormone metabolites, which show not just what your body is producing, but how it’s clearing and excreting estrogen, cortisol, and thyroid hormone once they’ve done their job.
This is the difference between a brief snapshot and a full picture.
What Your Symptoms Are Actually Telling You
When I look at someone who feels terrible despite normal labs, I am almost always looking at some combination of the following:
Impaired drainage. The liver is congested, the lymphatic system is stagnant, and the colon isn’t clearing efficiently, toxins and hormone metabolites that should be leaving are recirculating, and the body is essentially running in an environment it cannot clean.
Nutrient depletion at the cellular level. This is typically not flagged on standard labs, but is often the result of years of stress, hormonal contraception, gut dysfunction, or simply a modern diet that doesn’t replenish what the body is spending.
Mitochondrial dysfunction. Your cells are not producing energy efficiently, which shows up as exhaustion that sleep doesn’t fix, brain fog that coffee doesn’t lift, and a body that feels like it’s running on three cylinders.
Toxic burden. This is the cumulative load of what you’ve been exposed to over a lifetime and what your body has been storing because it couldn’t clear it fast enough.
Gut dysfunction. This is not necessarily diagnosed IBS or Crohn’s, but potentially just impaired digestion, dysbiosis, intestinal permeability, impaired absorption, and pathogens that make it so that the food you’re eating isn’t nourishing you the way it should.
None of these show up on a standard CBC or metabolic panel.
All of them are addressable.
What To Do With This
The answer is not to find a more aggressive doctor or to run every test imaginable.
The answer is to understand that the terrain, which is the internal environment your body is operating in, determines everything.
When the terrain is clear, the body regulates.
When it’s congested, depleted, and burdened, it struggles, regardless of what the labs say.
If your labs are normal and you still feel terrible, the work is not to find the pathology.
The work is to clear the terrain.
